Symptoms of MDD can look different from one person to the next, and their intensity may fluctuate over the course of an episode. A formal diagnosis requires at least five of the following symptoms to be present during the same two-week period, representing a clear change from previous functioning, with at least one symptom being either depressed mood or loss of interest or pleasure. 

Persistent Low Mood

Feeling sad, empty, tearful, or hopeless most of the day, nearly every day, often without a clear external cause.

Loss of Interest or Pleasure

A marked reduction in or complete absence of interest in activities once enjoyed, including hobbies, socializing, and intimate relationships.

Thoughts of Death or Suicide

Recurrent thoughts about death, passive wishes not to be alive, or active thoughts of suicide. These symptoms require immediate professional attention.

Sleep Disturbances

Insomnia, frequent waking during the night, difficulty returning to sleep, or hypersomnia, sleeping excessively without feeling rested.

Appetite or Weight Changes

Significant increase or decrease in appetite, often resulting in noticeable weight loss or weight gain within a relatively short period.

Difficulty Concentrating

Trouble thinking clearly, maintaining focus, making decisions, or retaining information, even on straightforward tasks.

Feelings of Worthlessness or Guilt

Excessive or disproportionate guilt, harsh self-criticism, or a pervasive sense of being a burden to others or to the world.

Physical Symptoms

Unexplained aches, pains, headaches, or gastrointestinal issues with no identifiable physical cause, which can sometimes be the most prominent presenting complaint.

Types

Types of Major Depressive Disorder We Treat

MDD is not one-size-fits-all. While all presentations share the core feature of depressive episodes, they differ in pattern, severity, timing, and associated features. Our psychiatric providers are experienced in diagnosing and treating all major forms and specifiers of MDD.

MDD, Single Episode

A first and, to date, only major depressive episode. Prompt and effective treatment following a first episode significantly reduces the likelihood of recurrence.

MDD, Recurrent

Characterized by two or more distinct depressive episodes separated by a period of at least two months with no significant depressive symptoms. Recurrent MDD often requires longer-term maintenance treatment to prevent future episodes.

MDD with Anxious Distress

A common specifier in which significant anxiety symptoms, such as tension, worry, and fear of losing control, accompany the depressive episode, often requiring a tailored treatment approach.

MDD with Melancholic Features

A more severe presentation characterized by profound loss of pleasure, early morning awakening, marked psychomotor changes, excessive guilt, and a depression that feels qualitatively different from ordinary sadness.

MDD with Seasonal Pattern (Seasonal Affective Disorder)

Depressive episodes that follow a predictable seasonal pattern, most commonly onset in late fall or early winter with remission in spring. Light therapy is often an effective component of treatment.

Treatment-Resistant MDD

What Causes Depression?

MDD rarely has a single identifiable origin. Research consistently points to an interplay of neurobiological, genetic, psychological, and environmental factors. Understanding these contributors is not about assigning blame; it is about building a treatment approach that addresses the whole person.

A family history of depression or other mood disorders increases the risk of developing MDD, reflecting a meaningful hereditary component.
